Parents and Guardians Tdap

Attention Parents and Guardians

The State of Illinois has mandated that all students entering, advancing or transferring into grade 9, 10, 11, and 12 must submit proof of receipt of Tdap vaccine for the 2013-14 school year.  Students who do not comply are subject to exclusion from school.

This requirement is important because Pertussis (whooping cough) disease continues to occur throughout many Illinois communities. Vaccination of children and adolescents reduces illness and absenteeism among school age students. In addition Pertussis in infants can be fatal; a vaccinated community helps protect infants, especially those who are too young to be fully vaccinated.

If you have received a letter and immunization record from Health Services please contact your physician or health care provider to fulfill this requirement.

Free immunizations offered at: Cook County Dept. of Public Health - Bridgeview Court House: Call for appointment: 974-6160

You must submit written proof to Health Services in room 142 or fax it to 708-728-3216.
